When checking out put the following information in the Notes block for us.
- Your name as it is on your chapter and national's roster.
- Your chapter and university
- Semester and year you joined
Please be aware that these certificates are intended for Member of Omega Delta Sigma only. We will check the rosters for your name, if it is not there we will not make a certificate for you.
